What the CNSS form shows
Form N128 separates employee and employer information and includes a registration request. The company’s affiliation number is not your personal number. Use the CNSS form rather than an unattributed shared template.
With HR — Barka advice
Look for an old card or certificate if you have worked before. Ask HR whether to recover an existing number or make a first application, then check name, ID and start date. Keep a copy and ask how receipt can be tracked.
If you get no answer
Take your ID, employment evidence and employer correspondence to a CNSS office. Ask which procedure fits your situation; a verbal promise is not proof that registration was processed.
